New Patient Paperwork

Welcome to ProFlex Physical Therapy. Thank you for giving us the opportunity to care for your physical and/or occupational therapy needs. We look forward to helping you in every way we can.

Please remember to bring your doctor’s prescription/referral, insurance referral if required from your insurance carrier, Operating Report, your insurance card, a photo ID, completed paperwork (forms can be downloaded – see above), and any MRI/CT/X-ray reports. It’s important to wear comfortable clothing, or you may bring a change of clothing if you prefer.

To expedite your initial visit, please fill out your paperwork for your physical and/or occupational therapy appointment. You can submit this paperwork via email to info@proflexpt.com or print and bring the completed forms with you to your first visit.
